New York Statutory DBL

• Over 50 lives-three years of premium, claims history

• Enriched DBL requires census with salary, gender, and DOB

• Employer paid, contributory or voluntary
• Family status – S, E&S, E&C or FF
• Prior carrier- include plan design and rates

Traditional Life with AD&D

• Census-include DOB, gender, salary, class and retiree(s) indicate if applicable, elected option for supplemental life-
Include in-force plan
